TRT the Right Way: Why a Prescription Is Not Enough

The testosterone replacement therapy industry has exploded. Online clinics advertise direct-to-consumer testosterone prescriptions with the ease of ordering a pizza. Fill out a questionnaire, do a single blood draw, get a vial in the mail.

It is fast. It is convenient. And for a lot of men, it is dangerously incomplete.

I have spent more than a decade managing hormone optimization protocols. Hundreds of patients, thousands of lab panels. So let me be direct: testosterone replacement therapy can change a man's life when it is done correctly. Men with clinically low testosterone who receive proper treatment often report improvements in energy, mood, body composition, and quality of life.

But "proper treatment" is the operative phrase. A prescription is the beginning of the process, not the end. Most of the TRT industry treats it as the end.

The TRT Mill Problem

The typical online TRT clinic runs a version of this playbook:

  1. You fill out an intake form online
  2. You do a single blood draw, often just total testosterone and maybe a CBC
  3. A physician you will never meet reviews your labs for about 90 seconds
  4. If your testosterone is below a threshold, and sometimes even if it is not, you receive a prescription
  5. You inject at home and reorder when you run out
  6. Maybe you get labs again in six months. Maybe.

This model exists because it is profitable, not because it is good medicine. The consultation is minimal. The monitoring is minimal. The understanding of your individual biology is nonexistent.

The consequences are real. I have watched men walk into my office after 18 months on an unsupervised protocol with hematocrit in the danger zone, estradiol through the roof, and no idea anything was wrong. They thought they were fine because nobody told them otherwise.

What Can Go Wrong With Unsupervised TRT

Testosterone is a powerful hormone. When you introduce exogenous testosterone, you set off a cascade of physiological changes that go far beyond "more energy."

Estradiol Spikes

Testosterone converts to estradiol through a process called aromatization. That is normal. Men need some estrogen for bone health, cardiovascular function, and brain health.

The problem shows up when nobody is watching the numbers. As testosterone climbs, aromatization increases. Unchecked estradiol can cause water retention, gynecomastia, mood swings, and reduced effectiveness of the testosterone itself.

A competent physician monitors estradiol and adjusts the protocol. Sometimes that means lowering the dose. There is an ongoing debate in the endocrine community about aromatase inhibitors; many clinicians now prefer dose adjustment alone, while others use them selectively. The point is that someone qualified needs to be reviewing your bloodwork regularly. TRT mills rarely check estradiol at all.

Elevated Hematocrit

I have seen this one catch men off guard more than any other side effect.

Testosterone stimulates erythropoiesis, the production of red blood cells. That is one reason TRT improves energy and endurance. Unchecked, it can push your hematocrit (the percentage of blood that is red blood cells) to levels that thicken the blood.

The Endocrine Society clinical practice guideline (Bhasin, Journal of Clinical Endocrinology and Metabolism, 2018) recommends against starting testosterone in men with elevated hematocrit and calls for measuring hematocrit as part of standardized monitoring during the first year of treatment. Erythrocytosis is the most common laboratory abnormality seen on testosterone therapy, reviewed in detail by Ohlander and colleagues (Sexual Medicine Reviews, 2018).

Proper monitoring means checking hematocrit on a schedule your physician sets and acting when it trends upward: adjusting dose, changing delivery method, or recommending therapeutic phlebotomy. Without monitoring, you will not know there is a problem until you are in an ER.

PSA and Prostate Health

Let me address a persistent belief: that testosterone causes prostate cancer. Morgentaler and Traish (European Urology, 2009) reviewed the experimental and clinical literature from 1941 to 2008 and proposed the saturation model, in which prostate tissue's response to androgens plateaus at relatively low testosterone concentrations. That reframed the old assumption. What remains true is that testosterone can support the growth of existing prostate tissue, and the Endocrine Society guideline (Bhasin, 2018) recommends against starting testosterone in men with prostate cancer, a palpable prostate nodule, or an elevated PSA without further urological evaluation.

Baseline and ongoing PSA monitoring is non-negotiable for any man on TRT. A rapid PSA rise does not necessarily mean cancer, but it demands investigation. Any provider who is not tracking PSA is gambling with your health.

Fertility Suppression

This is the one that blindsides younger patients.

Last year I had a 32-year-old come in. He had been on TRT from an online clinic for almost two years. He and his wife were trying to conceive. Nobody had mentioned that exogenous testosterone suppresses your body's natural production through the hypothalamic-pituitary-gonadal (HPG) axis. His brain sensed adequate testosterone and stopped signaling his testes to produce it. Testicular atrophy. Sperm count near zero.

This is well documented. Hsieh and colleagues (Journal of Urology, 2013) note that testosterone replacement impairs spermatogenesis and leads to azoospermia in about 40% of patients; in their series of 26 men, adding low-dose human chorionic gonadotropin (hCG) alongside testosterone preserved semen parameters. The Endocrine Society guideline (Bhasin, 2018) recommends against starting testosterone at all in men planning fertility in the near term.

We got my patient back on track with hCG and a carefully managed recovery protocol, but it took almost eight months. It could have been avoided if his first provider had asked one question: "Do you want kids?"

Many online clinics never bring it up.

Lipid Changes

TRT can shift your lipid profile, sometimes dropping HDL while nudging LDL up. In men who already have suboptimal lipids, that is a slow-building cardiovascular risk that compounds quietly over months and years. No symptoms. No warning. Just numbers drifting in the wrong direction while nobody checks.

What the Cardiovascular Evidence Actually Says

For years the honest answer on TRT and heart risk was "we do not know." That changed with TRAVERSE (Lincoff, New England Journal of Medicine, 2023), a randomized, double-blind, placebo-controlled trial of 5,246 men aged 45 to 80 with symptoms of hypogonadism, two fasting testosterone levels under 300 ng/dL, and existing or high risk of cardiovascular disease. Men received daily transdermal testosterone gel, dosed to keep levels between 350 and 750 ng/dL, or placebo, with a mean follow-up of 33 months.

The result: a major adverse cardiac event occurred in 7.0% of the testosterone group and 7.3% of the placebo group (hazard ratio 0.96). Testosterone was noninferior to placebo on the primary endpoint. The same trial reported a higher incidence of atrial fibrillation, acute kidney injury, and pulmonary embolism in the testosterone group.

Two takeaways. First, in men who actually meet criteria and are treated to physiologic levels under monitoring, the feared cardiac signal did not appear. Second, the secondary findings are exactly why monitoring matters. This was a supervised trial with dose adjustment and lab follow-up. It says nothing about a vial in the mail and a lab draw twice a year.

What Proper Hormone Optimization Actually Looks Like

So what separates physician-supervised TRT from the mill model? I break it into three phases.

Phase 1: Comprehensive Baseline Assessment

Before prescribing anything, I need your complete hormonal picture. Not a single testosterone number. The full map:

  • Total and free testosterone, measured fasting in the morning and confirmed on a repeat draw, which is exactly what the Endocrine Society guideline (Bhasin, 2018) recommends for diagnosis
  • Sex hormone-binding globulin (SHBG): this protein binds testosterone and makes it unavailable; high SHBG can cause low free testosterone even with normal total levels
  • Estradiol (sensitive assay): baseline estrogen before introducing testosterone
  • LH and FSH: to determine whether low testosterone is primary (testicular) or secondary (pituitary)
  • Prolactin: elevated levels can suppress testosterone and may indicate a pituitary issue
  • Complete blood count: baseline hematocrit and hemoglobin
  • Comprehensive metabolic panel: liver and kidney function
  • Lipid panel: baseline cardiovascular markers
  • PSA: baseline prostate health
  • Thyroid panel: thyroid dysfunction often mimics or compounds low T symptoms
  • DHEA-S, cortisol: adrenal function, which affects hormone balance

That is 20+ markers before a single prescription gets written. Compare that to the one or two tests most online clinics bother with.

Phase 2: Personalized Protocol Design

Whether to treat at all is a physician decision, made with you, after the labs and the symptoms line up. Availability of testosterone treatment also varies by state, and a legitimate program will tell you up front whether it can prescribe where you live.

If treatment is the right call, your labs, symptoms, goals, age, and health history all feed into a protocol designed for you. Not a template. You. That includes delivery method (injections, topical gel, or pellets, each with different pharmacokinetics), dosing frequency (weekly, twice-weekly, or more frequent smaller doses to keep levels stable), any ancillary medications, and the lifestyle changes that support what the hormones are doing.

No single TRT protocol works for every man. I have had two patients with nearly identical total testosterone numbers respond completely differently to the same protocol. One did well on twice-weekly injections; the other did not feel right until we switched to a daily topical. Biology is individual. Your protocol should be too.

Phase 3: Ongoing Monitoring and Adjustment

This is where everything either comes together or falls apart.

The Lifestyle Piece Most Providers Ignore

Something that frustrates me about this industry: testosterone therapy works better when paired with the right lifestyle inputs. And almost nobody talks about it.

Sleep is the clearest example. Leproult and Van Cauter (JAMA, 2011) restricted ten young healthy men to five hours of sleep a night for one week and measured a 10% to 15% drop in daytime testosterone. Small study, but a striking effect. If you are on TRT and sleeping five hours a night, you are fighting your own biology.

Body composition matters too. Adipose tissue contains aromatase, the enzyme that converts testosterone to estradiol. More body fat means more conversion. I can show you the difference on bloodwork between two patients on the same dose with very different body fat percentages.

Resistance training and basic micronutrient status, zinc, magnesium, vitamin D, adequate dietary fat, all support how your body uses hormones. Deficiencies blunt even a well-designed protocol.

How to Evaluate Any TRT Provider

Whether you end up working with us or somebody else, ask these questions before you start:

How many biomarkers do you test at baseline? We run 20+. Fewer than a full hormonal, metabolic, and prostate baseline and your provider is flying partly blind.

How often do you recheck labs? The answer should be a specific schedule set by a physician, with more frequent draws early. If your provider waits six months or more between draws, problems can develop and compound without anyone noticing.

Do you monitor estradiol, hematocrit, and PSA on every panel? These are the three markers where unsupervised TRT causes the most damage. If even one is missing from the standard panel, that is a deal-breaker.

Will I see the same physician each time? Continuity matters. A new doctor every visit means nobody is tracking your trajectory, and in hormone management, trajectory is everything.

Do they address sleep, nutrition, and training? If testosterone is all they offer, the protocol is incomplete.

What happens if you want to preserve fertility? If the provider seems surprised by this question, that tells you everything about their depth of practice.

Can you prescribe in my state? Availability varies by state. A legitimate program will tell you before you pay.

Frequently asked questions

Is testosterone replacement therapy safe long-term?
The best evidence to date is TRAVERSE (Lincoff, New England Journal of Medicine, 2023), where testosterone gel was noninferior to placebo for major adverse cardiac events over a mean of 33 months in men with low testosterone and elevated cardiovascular risk, with higher rates of atrial fibrillation, acute kidney injury, and pulmonary embolism in the testosterone group. That is a strong argument for treating men who meet criteria and a strong argument for monitoring. Long-term safety for any individual depends on proper selection, dosing, and follow-up, which is a physician's job.

How do I know if I actually need TRT?
Low testosterone symptoms, fatigue, brain fog, low libido, loss of muscle, low mood, overlap with a dozen other conditions. That is why comprehensive lab work comes first. Thyroid dysfunction, sleep apnea, chronic stress, and nutrient deficiencies can all mimic low T. The Endocrine Society guideline (Bhasin, 2018) calls for a fasting morning total testosterone, confirmed on repeat, plus symptoms, before a diagnosis. Whether to treat is a physician decision.

What is the difference between TRT from an online clinic and a physician-supervised program?
Scope of care. Online clinics typically test one or two markers, prescribe testosterone, and check in sporadically. Can I still have children while on TRT?

How quickly will I see results from TRT?
Many men notice changes in energy and mood within the first several weeks, and body composition changes take longer. Optimization is a process; it usually takes two or three rounds of labs and adjustments to dial things in. Individual results vary. Anyone promising an overnight transformation is not being straight with you.

The Bottom Line

Testosterone replacement therapy is one of the most useful tools available for men with real, measurable hormonal decline. Done right, under a physician, with full labs and ongoing monitoring, it can restore energy, sharpen thinking, change body composition, and improve quality of life.

Done carelessly, it creates new problems while half-solving old ones.

A prescription is a tool. Optimization is a process. Do not settle for the tool without the process.
